Leaving Care Social Worker 16+

Caerphilly

£37.97 per hour Umbrella company

We are currently looking for a Leaving Care Social Worker to join Caerphilly Social Services. The contract will initially be for 6 months with a view to extend. Pay rate for this position is £37.97 per hour subject to experience, payrolled via an Umbrella company (PAYE options available). This role offers hybrid working, the on site base is in Ystrad Mynach.

The team are seeking an enthusiastic social worker to assist in supporting their young people who range in age between 16 to 25. The team works with young people who are looked after, homeless and in need of accommodation as well as unaccompanied asylum seekers. The team is made up of experienced social workers who can support you in your role and support you with building strong connections to wider services to assist us in the support you provide the young people.

We seek social workers who can:

  • Develop clear plans [Pathway Plans] to aid young people to develop their independence and confidence as they reach adulthood
  • Provide high quality assessment and/or care management to children and families who have been assessed as requiring ongoing intervention
  • Review intervention within statutory timescales ensuring plans for children are achieving the identified outcomes
  • Demonstrate professional competence in social work practice.
  • Contribute to service development initiatives
  • Ensure effective multidisciplinary working with partner agencies
  • Represent the service at Court and/or Panel meetings
  • Manage and be accountable, with supervision and support, for your own social work practice
  • Assess need in accordance with Directorate policies, procedures and relevant assessment frameworks, to include Working Together to Safeguard Children, and other locally and nationally agreed protocols
  • Develop effective, outcome focused intervention plans in consultation with children and young people themselves, and with their parents or carers, in order to safeguard their welfare, address risk and ensure that these plans are fully implemented and regularly reviewed
  • Maintain accurate case records in accordance with the Directorate policy, keeping such records up-to-date

To apply for this role, you will need to hold -

  • A minimum of 6 months experience of working in the UK as a registered social worker
  • A relevant Social Work Qualification
  • Registration with Social Care Wales / Social Work England
  • Enhanced DBS, registered on the update service or be happy to apply for a DBS
  • 3 Years of referencing 
  • Driving license with access to a car
