Community Mental Health Team Social Worker

South Gloucestershire Council

Thorbury

Pay rate up to £32.65 Umbrella company


2 Qualified Social Workers are required to join South Gloucestershire's CMHT based out of The Elms, Thornbury. Ideally we are looking for 2 full time candidates but part time applications will be considered. Initial 3 month contracts offering hybrid working. Pay rate for these positions is £26 PAYE, and £32.65 Umbrella company.

Duties to be undertaken within this post include the following:

a) To carry out assessments and reviews within the framework of the Care Act 2014 for service users new to the service and to review care packages for those already receiving care packages, to make sure that the agreed plans are implemented.

b) To ensure that care packages continue to meet previously assessed needs and where they no longer do so to reassess and either1) decrease the care package and so avoid disempowerment of service users or 2)increase them with a view to preventing or delaying needs.

c) You will have a working knowledge of the Mental Capacity act and be able where necessary to advise other colleagues in the team in relation to the process of carrying out an assessment for capacity and the process of best interests’ decisions.

d) To operate in line with the council’s safeguarding policy and where necessary to raise safeguarding alerts in relation to service users’ needs for intervention.

e) To carry out carer’s assessments in line with the Care Act 2014 to support them in their caring role.

f) You will carry a caseload which will include more complex cases and be expected to engage with this work from the start of your placement.

g) You will work for the North Recovery team based in Thornbury and your service users will be based in the north area of the local authority

h) To give advice, information and assistance to disabled people and carers so that they are aware of their rights and entitlements, and of the content of the care plan.

i) To ensure that the Team Manager/Senior Practitioner is kept fully informed of the service users changing needs so that the care plan is properly updated in the light of the service users’ needs.

j) To establish and maintain effective contact with service users, their relatives, Managers and staff of service providers and colleagues in other agencies in order that there is full awareness of service users’ needs and wishes.

k) To assist with the induction of new staff, in accordance with induction plans and agreed with the Team Manager, to help ensure the implementation of high quality, informed practice.

l) To maintain written records in accordance with departmental guidelines, including inputting information to and extracting data from computer systems relevant to the job so that the recording and monitoring of care plans is properly implemented.

m) To work together with service providers to assist in care planning with other agency and non-agency providers to access services as identified within the care plan.

n) To be aware of their responsibilities for the 1974 Health and Safety at Work Act.

o) To assist in the completion of financial assessments of all service users to enable the calculation of care charges and ensure relevant financial procedures are complied with.

p) Be required, as part of a team, to participate in providing information and assistance to telephone and visitor enquiries on a rota basis.

q) To operate both the South Glos council and the AWP trust computer systems.

r) You will not be participating in the Council’s AMHP rota, even if you are a qualified AMHP.

 

To apply for this role, you will need to hold -

  • A minimum of 6 months experience of working in the UK as a registered social worker
  • A relevant Social Work Qualification
  • Registration with Social Work England
  • Enhanced DBS, registered on the update service or be happy to apply for a DBS
  • 3 Years of referencing 
  • Driving license with access to a car
